5mg Ramipril/25mg Hydrochlorothiazide

A combination of angiotensin-converting enzyme (ACE) inhibitor and thiazide diuretic used to treat high blood pressure

Oral  Usual dose:

Ramipril 2.5 mg/hydrochlorothiazide 12.5 mg once daily. Titrate as needed to a maximum dose of Ramipril 10 mg/hydrochlorothiazide 50 mg once daily. Note: Patients receiving other diuretics prior to initiation of Ramipril/hydrochlorothiazide therapy should attempt to discontinue diuretic therapy at least 2 to 3 days prior to initiation of treatment, or at least reduce the dose of the other diuretic; if discontinuation of other diuretics is not possible, Ramipril should be initiated at the lowest possible dose (e.g., 1.25 mg daily)

Precautionary tips

– Do not give to patients with hypersensitivity to ramipril and hydrochlorothiazide. – Do not give to patients with hereditary/idiopathic angioedema.
– Do not give to patients with bilateral or unilateral renal artery stenosis.
– May cause anemia because ACE inhibitors can inhibit erythropoietin production. This is more likely to occur in the presence of chronic kidney disease.
– Non-immune anaphylactic/anaphylactoid reaction can occur with ACE inhibitors.
– Symptomatic hypotension with or without syncope can occur (usually with the first several doses).
– Use with caution in patients with severe aortic stenosis. – Avoid use in patients with ascites due to liver cirrhosis.
– Use with caution in patients with collagen vascular disease. – Use with caution in patients with hepatic insufficiency.
– Use with caution in patients with hypertrophic cardiomyopathy and left ventricular outflow tract obstruction.
– Use with caution in patients with renal insufficiency.
-ACE inhibitors may cause a dry cough that usually occurs within the first few months of treatment and generally resolves within 1 to 4 weeks after stopping ACE inhibitors.
 -ACE inhibitors may cause hyperkalemia. 
Pregnancy and lactation
– Drugs that act on the renin-angiotensin system can cause fetal harm. Ramipril crosses the placenta. Exposure to angiotensin-converting enzyme (ACE) inhibitor during the first trimester of pregnancy may be associated with an increased risk of fetal malformations. Drugs that act on the renin-angiotensin system cause oligohydramnios. When treating hypertension during pregnancy, ACE inhibitors should generally be avoided.
– Thiazide diuretics and ramipril are excreted in breast milk. Therefore, this combination should not be used by nursing mothers.
